Capillaritis

A benign condition with chronic, non-palpable, cayenne-pepper-like petechial macules on the lower legs, which do not indicate a systemic disease.

Thrombocytopenia

A low platelet count causing non-palpable petechiae and ecchymoses (bruises) that are not associated with inflamed vessels.

Vasculitis

Inflammation of blood vessels causing characteristic palpable purpura (raised, non-blanching red/purple spots) from extravasated red blood cells.
