Acrodermatitis enteropathica

A rash from zinc deficiency presenting with a characteristic periorificial and acral distribution of eczematous and vesiculobullous lesions, which is severe in the diaper area.

Allergic contact dermatitis

An eczematous rash with sharp cut-offs corresponding to the area of contact with an allergen in a diaper or wipe.

Atopic dermatitis

Classic eczema which, contrary to irritant dermatitis, often spares the diaper area because the occlusion from the diaper is protective.

Bullous impetigo

A superficial bacterial infection causing flaccid, clear bullae that rupture easily, commonly seen in the moist diaper area.

Candidiasis

A yeast infection presenting as a beefy red, moist rash with characteristic small satellite papules and pustules at the border.

Congenital syphilis

Can present with a widespread vesiculobullous or papulosquamous eruption that includes the diaper area, along with other systemic signs.

Granular parakeratosis

A rare condition with erythematous to hyperpigmented, keratotic papules that coalesce into a rough, macerated plaque in intertriginous areas, including the diaper area.

Irritant dermatitis

The most common type, caused by friction and moisture, presenting as erythema and erosions on the convex surfaces while characteristically sparing the skin folds.

Langerhans cell histiocytosis

A rare neoplastic disorder that can present as a scaly, seborrheic dermatitis-like eruption in the diaper area, often with petechiae or purpura.

Perianal streptococcus

A bacterial infection presenting as a sharply demarcated, bright red erythema specifically around the anus.

Psoriasis

Presents as well-demarcated, bright red, shiny plaques that may lack scale due to moisture, sometimes called 'napkin psoriasis'.

Seborrhoiec dermatitis

Presents as erythematous patches with greasy scale, often involving the scalp ('cradle cap') and skin folds, including the diaper area.
