Erythema multiforme

Characterized by classic 'target' lesions, often acrally distributed and frequently triggered by herpes simplex infection, with less severe mucosal involvement than SJS.

Herpes simplex

Widespread infection (eczema herpeticum) can cause erosions, but is distinguished by a positive viral culture or PCR and lacks the dusky, atypical targets of SJS.

Mucous membrane pemphigoid

A chronic autoimmune blistering disease primarily affecting mucosal surfaces, leading to scarring, unlike the acute, drug-induced nature of SJS.

Pemphigus

An autoimmune blistering disease causing flaccid bullae and erosions due to autoantibodies against desmogleins, confirmed by immunofluorescence.

Toxic epidermal necrolysis

Considered a more severe variant of the same disease process, distinguished by a larger body surface area of epidermal detachment (>30%).
