Necrobiotic xanthogranuloma

A rare condition associated with paraproteinemia, presenting as indurated, yellowish plaques and nodules that often ulcerate, typically in the periorbital region.

Palpebral sarcoidosis

Presents as firm, reddish-brown or yellowish papules and plaques on the eyelids, and biopsy reveals non-caseating granulomas.

Plane warts

Flat-topped, skin-colored papules caused by HPV, which are not yellowish and do not represent lipid deposition.

Sebaceous gland hyperplasia

Yellowish papules on the face, but these are smaller, more numerous, and have a characteristic central umbilication.

Seborrhoiec keratoses

Can be yellowish but are distinguished by their warty, 'stuck-on' surface texture.

Syringomas

Small, firm, skin-colored or slightly yellow papules, typically on the lower eyelids, which are benign sweat duct tumors and are much smaller and firmer than xanthelasma.
